Health Coaching 5 Session Package
Package includes: 5, 45-minute telephonic sessions at a 10% discount. Sessions are not transferable.
In addition, supplemental materials such as handouts, assignments, tools, videos or TED talks may be emailed to you. An in-between session text to maintain accountability will be discussed at the end of your session. This additional help will assist you in achieving your goals and help you maintain a healthy lifestyle.
Please download and fill out the Health Coaching Agreement and return prior to your session. If you have already filled out this form with your initial session, you do not need to resend it. You’ll receive a 30-minute telephonic coaching session. This is a follow-up session and not intended for new clients. If you are new client, please select the new client session.
5 session follow-up packages: You will receive 5, 30-minute telephonic sessions for a 10% discount. You are encouraged to commit to 3 months of sessions to make behavioral changes needed to change a lifestyle. Remember, what you do between sessions is vital to your success!
In addition, supplemental materials such as handouts, worksheets, TED talks, articles or tools will be emailed to you and you’ll receive a mid-session check before your next session to maintain accountability. This additional help will assist you in achieving your goals and help you maintain a healthy lifestyle.
I will call you at the number of your choice within 5 minutes of the scheduled time. I’ll send you a mid-session touch base to keep you accountable and on track. I will follow-up via text or email (we will decide this at the end of the coaching session)
If you need to cancel and reschedule, please do so at least 24 hours in advance.
